We represent over a hundred primary care providers dedicated to ensuring high quality integrated care for the communities of Markham, Stouffville and Uxbridge that we serve.

Our VISION

Within the larger context of the Ontario Health Team initiative, our mission is to ensure that all family physicians and nurse practitioners within our community, regardless of practice type, have access to tools and resources to offer seamless care.
