In collaboration with people with lived experience, caregivers, residents, primary care, community service providers, long term care homes, retirement homes, Markham Stouffville Hospital, York Region and Durham Region, the EYRND OHT was launched with the vision of providers working as one coordinated team to meet the needs of our community.
