who can join?
You can join our Primary Care Network if you are a family physician or primary care nurse practitioner who serve patients in Markham, Stouffville or Uxbridge. You may belong to more than one Ontario Health Team or Primary Care Association.
Patients, caregivers and those with lived experience who want to help shape our local healthcare system should contact the Eastern York Region North Durham Ontario Health Team.

Joining our Primary Care Network gives you a voice in shaping our Ontario Health Team as we strive to provide seamless coordination of care for your patients in a way that supports the work of primary care providers. Our Primary Care Network, through our elected Primary Care Advisory Council represents you in the collaborative work of OHT partners including hospitals, patients, caregivers, and community organizations.
Primary Care Network members will have access to regular email updates, a member web-portal and periodic webinars. You will have to opportunity to participate in Ontario Health Team initiatives and elections of your Primary Care Advisory Council representatives.
