Seamless Care Optimizating the Patient Experience (SCOPE)

SCOPE provides a shared virtual inter-professional care team for family physicians and a platform for primary care engagement and connection with hospital and community care resources. Developed in 2012 with a pilot group of 30 solo primary care providers, the program has grown to over 300 providers across 5 Toronto sub-regions.

The EYRND OHT with our Primary Care Association and Oak Valley Health will launch SCOPE in 2022. If you are interested in participating in this local pilot, please email Dr. Cristina Popa (cpopa@msh.on.ca)



Seniors Home Support Program

The Seniors Home Support Program was launched in April 2021. It is an integrated primary care program delivered by an interdisciplinary team to home-bound seniors who for medical, social or cognitive reasons cannot access office-based primary care services.

The SHS Program is delivered by a care team which includes primary care physicians, a nurse practitioner, an occupational therapist, a pharmacist, a social worker and a chiropodist. Home-care services are provided in partnership with the Markham Family Health Team (FHT), Markham Stouffville Hospital (MSH), South East Geriatric Outreach Team of the New Unionville Home Society, Central Region Home and Community Support Services and York Region Paramedic Services Community Paramedicine Program.

 

Pandemic Response Planning

The EYRND OHT Primary Care Association continues to work with our partners in ongoing pandemic response. Thank you to everyone who has supported our efforts! The amazing collaboration within EYRND OHT has successfully supported the Cornell Vaccination Clinic, MSH Assessment Centre and Carefirst High Priority Communities Initiative.
