Two quick updates:
PPE
Markham Stouffville Hospital is offering another round of PPE procurement for OHT primary care physicians. The hospital will invoice the physician offices on a one time basis and contact you to coordinate pick up upon arrival. Fill out form here to order: https://www.surveymonkey.com/r/K2C9HGJ. The deadline for ordering PPE is July 6, 2020.
Dementia Care
FirstLink is a program offered by both the Alzheimer's society of York and Durham region. The program provides support and information for patients and caregivers from early diagnosis and throughout the dementia journey. They also provide care navigation and links to services. Attached is the referral form for FirstLink and I would encourage you to refer all patients to this program right from early diagnosis of dementia. If you have feedback on this program, please let me know and I'll ensure the feedback is received by the FirstLink program directors.
The Alzheimer Society of Ontario has developed an online education course for primary care related to the diagnosis, treatment and interventions related to dementia. There is a webinar that provides an overview of the care pathways and the platform for the online course. The same webinar is offered twice, see below for dates and times. Attached is the flyer with more information.
