Community Support Services
Hi everyone,
I’d like to highlight some community supports that are available to our vulnerable patients, including the elderly, caregivers, marginalized populations and those with complex care needs or are suffering from illness. Also below is information about a local meeting this afternoon with Southlake cardiologists on how to best manage our cardiac patients during COVID.
EYRND OHT Community Support Services
The OHT partners have launched a new community support services initiative to centralize intake for essential services to our vulnerable populations in Markham, Stouffville, Thornhill, Uxbridge and Brock. Services include providing local transportation to medical appointments, telephone security and wellness checks, social work and counselling services as well as caregiver support services, Meals on Wheels, grocery and pharmacy pick-up and delivery, personal support and homemaking services. Patients can self-refer by calling 1-877-452-4287 or going online to https://eyrnd-oht.covidresponse.ca/.
Patients will be connected with services from one of the participating agencies which include Alzheimer Society of Durham Region, Alzheimer Society of York Region, Brock Community Health Centre, Carefirst, CHATS – Community & Home Assistance to Seniors, Community Care Durham, Durham Region, Entité 4, Krasman Centre, LOFT Community Services, Markham Stouffville Hospital, Region of York, SE Health, and York Support Services Network. Evergreen Anticipatory Grief & Bereavement Support
For those who require anticipatory grief and bereavement support, Evergreen provides services in Thornhill, Markham and Stouffville including
• Virtual Professional Anticipatory Grief Counselling to support client who is ill and/or their family caregivers
• Caregiver resources and mailings
• Virtual Support by trained volunteers by phone
• Virtual Bereavement counselling
• Bereavement information mailings
• Virtual Support groups for those whith Cancer
