Opportunities for Primary Care Engagement

Dear OHT Primary Care Providers,

I am excited to present you with opportunities to help shape our OHT as we move forward towards a more integrated healthcare model. We are seeking the knowledge, experience and expertise of primary care providers to help build our OHT. We aim to have a primary care voice at each and every OHT table.

Attached is a list and description of action teams and committees that are recruiting primary care members. The only mandatory requirement for application is that you are a family physician or nurse practitioner who provides care for patients in the EYRND OHT catchment area (Markham, Stouffville, Uxbridge and Brock). The time commitment is expected to be 2-3 hours per month of meetings, which are usually scheduled during regular business hours and well in advance. An honorarium of $500 per quarter will be provided to acknowledge your commitment and leadership.

If you are interested in more than one of the opportunities, you can submit a single application and check off the action teams or committee you are interested in. Our Primary Care Advisory Council will evaluate all applications and make selections based on merit and balance of representation.
