OHT Updates
- The OHT website https://www.eyrnd.ca/ is officially launched! Please let me know if there is any feedback.
- The OHT along with partners in LTC, the Ontario Retirement Communities Association and Markham Stouffville Hospital are launching a LTC/RH - ED Transfer Communication Tool in October, 2020. Residents from Health For All Family Health Team will be involved in a QI project to evaluate this initiative
- Today ended an intensive 2 day design session to build an integrated primary care program for house-bound seniors with partners from Central & Central East LHIN, York Region, New Unionville Home Society, Regional Geriatric Program, Carefirst FHT, Health for All FHT, Markham FHT and York Region EMS. I will have more to share as we move forward with this work.
- I will have an update on the ARI clinic and flu season strategy at the end of next week
